1-day WORKSHOP for the CHALLENGE

This interactive workshop will focus on expanding participants’ thinking and enhancing clinical skills in assessment of advanced motor skills (Challenge) of children and youth who have CP.
What is the Challenge?
Motor measures currently used by paediatric physiotherapists do not fully capture advanced motor skill deficits in children in GMFCS I (i.e., able to run and jump independently but limited in speed and quality of movement). This gap may lead to serious evaluation issues, such that lack of understanding of their physical abilities/challenges may mean missed intervention opportunities to help these children achieve their gross motor and participation goals. The Challenge (Wilson et al., 2011; Glazebrook et al., 2013; Wright et al.,2017; Gibson et al.,2017) is an observational measure that assesses 25 advanced gross motor skills that tap into a child’s co-ordination, agility, balance, fitness and strength. It was developed by our research team to address this measurement gap. It can be used with children with cerebral palsy who are age 6 years and up and are in GMFCS Level I, and also with higher ability children in GMFCS II (i.e., able to walk and run slowly without an assistive device). A 20-item Rasch scaled version is also now available for use (Wright et al.,2016).
Who is the course for?
This workshop will be of interest to paediatric physiotherapists who have clinical experience with using the Gross Motor Function Measure or other gross motor tests with ambulatory children with CP (those in GMFCS Levels I, II and III) and want to learn additional ways of assessing the gross motor movement of these children. Paediatric occupational therapists who actively use gross motor tests in their clinical practice or research may also be interested in taking part.

Challenge workshop objectives
- Review the current methods measuring advanced motor skills in children with CP
- Learn about the content and structure of the Challenge and its potential uses
- Become familiar with the Challenge’s generic and item-specific scoring system
- View the performance of the Challenge by children with CP (videos) and apply Challenge scoring
- Learn the process of administering and scoring a Challenge assessment through live demonstration by instructor with typically developing children (an 11 to 12 year old, and a 6 to 7 year old to show item adaptations to simplify for younger children) and then a child with CP.
- Acquire tips for implementing the Challenge in clinical and research practice
- Review the next steps of self-training using the Challenge video materials posted for exclusive use of course participants on the Holland Bloorview website.
- Outcomes website in preparation of Challenge Criterion Test (also posted on the password protected website)
The ABI-Challenge Assessment (ABI-CA) for use with Children with Brain Injury
Also included in the Challenge workshop will be an opportunity to learn about an exciting new outcome measure, the ABI-CA which was designed by the Holland Bloorview clinical research team for use with children over the age of 6 years with mild to moderate ABI and are either in the later stages of in-patient rehabilitation or at home in their community (Ibey et al.,2010; MacArthur et al., 2013; Wong et al.,2014). It is based on exactly the same system of dynamic assessment as the Challenge for children with CP, and also measures advanced motor skill performance. Learning how to use the CP Challenge provides sufficient background for therapists to complete the on-line training and Criterion Test certification for the ABI-CA after the Challenge course.
Additional Challenge workshop (22 June 2018)
This will be an interactive workshop run by Virginia Wright to enable participants who have just completed the Challenge workshop to consolidate their skills and further prepare them towards scoring and video examples and gaining certification.
- Administer the Challenge in groups of 2 – 3 therapists to typically developing children and children with cerebral palsy (GMFCS I or II)
- Gain tips in how to administer the items and simplify them where necessary for individual children at different skill levels
- Improve precision of scoring through practice and feedback
The additional Challenge workshop will only be available to a small group of therapists. If you would like to attend this extra days training, please express your interest on the course registration form, limited availability.
How will the course be taught?
The workshop is a combination of lecture style sessions with group discussion during viewing and scoring of sample training videos. A hands-on component with children will provide opportunity for participants to observe and participate in live administration of the Challenge. After the course, self-study of on-line training videos (available only to course participants) will be done prior to completion of the in-line Criterion Test (certification)
NOTE: Certificates of attendance will be provided by Holland Bloorview Kids Rehabilitation Hospital. This Certificate implies neither the certification nor competence as an assessor on either test. On successful completion of the Challenge criterion test after self-study of the web-based materials, a Criterion Test Certificate will be issued.
Tutor: Virginia Wright, PT, PhD
Dr Wright is a physiotherapist and Clinician Scientist at Holland Bloorview Kids Rehabilitation Hospital, Toronto, Canada, and leads the SPARK (Supporting Physical Activity-based rehabilitation Research for Kids) Lab in the Bloorview Research Institute. Virginia holds the Bloorview Children’s Hospital Foundation Chair in Paediatric Rehabilitation (2012- 2017). She is a Professor in the Department of Physical Therapy at the University of Toronto and an Associate Member of CanChild, McMaster University, Hamilton, Canada. She has an MSc and PhD in Health Research Methodology from McMaster University, Hamilton, Canada.
Virginia’s research is grounded in her experience as a physiotherapist and outcome measures co-ordinator, and guided by her close links with several pediatric rehabilitation teams at Holland Bloorview. She currently holds 3 research grants as co-principal investigator (1.5 million dollars in research funding), and is co-investigator on 5 other grants ($800,000 in total). Her clinical research centres on development and validation of outcome measures for use in pediatric rehabilitation (cerebral palsy, prosthetics, and acquired brain injury in particular), and on evaluation of a variety of interventions in cerebral palsy. Her current program of gross motor outcomes research includes: validation of a new quality of movement measure (the Quality FM) that she created for children with cerebral palsy (CP) (as a companion to the Gross Motor Function Measure [GMFM]); refinement and validation of an advanced motor skills assessment (the Challenge Module) that she designed for children with CP; and a randomized trial that is evaluating treadmill-based robotic gait training in children with CP
